Candidates who have successfully completed the Senior Secondary Education are qualified to purchase the form with credit in at leat five (5) subjects, which must include:English Language, Mathematics, Biology, Chemistry, Physics, at not more than two sittings, in two accredited SSCE examination board. (i.e. NECO & WAEC).

NOTE: Combination of NECO & WAEC will be entertained.

Fill the Form with BLACK Pen and it should be submitted with the photocopy of the School Receipt, at the Principal’s Office on or before the closing date (10th June, 2016).
Note that attention shall be given to forms submitted on time.
